Health Clinics for school physicals

New students entering Oak Terrace need to have physical examination records on file in the nurse's office.
 
 

Children need to be fever free for 24 hours prior to returning to school after being absent.
 
 

All medication including over-the-counter products such as aspirin, cough drops, and cough syrup must be administered by the nurse.

All students entering 5th grade must have a physical examination and record of updated immunizations (including the three series hepatitis B vaccine) on file in the health office before the first day of school, August 27, 2002.  The physical exam must be within one year of the first day of school, ie: exams completed after August 27, 2001 ARE acceptable.  Start early and avoid the summer rush in the Doctor's office. 
 

Lake County Health Department holds immunization clinics at Highland Park Hospital on the 4th Friday of every month from 9:00 am-11:00 am  Cost is $8.00 each or as able to pay.  No appointment necessary. Please bring your immunization record with you.

Care Coach visits North Shore Estates  on the 2nd Tuesday of each month from 3:30-6:00 pm.   No appointment necessary.   All immunizations are FREE to Lake  County residents.  Please bring your immunization record with you.  You do not have to live at North Shore Estates to receive the service.  Visit their web site at www.lakeforesthospital.com
 

Lake County Health Department offers health clinics on the 1st Tuesday of each month from 9:00 am-11:30 am for school physicals at a cost of $18.00.  No appointment necessary, but get there early.  Immunizations are also available with a physical.

Canmann Well Child Clinic at Highland Park Hospital:   Get a school physical for only $5.00!   Meets the 4th Tuesday of every month BY APPOINTMENT ONLY.   Call Bev Crowley at 847-480-3727 to schedule an appointment.  No immunizations available at this clinic.
